To get this ending, you must complete the game on Punished Difficulty with a maximum of three healing items. One of these heals is spent automatically at the start, leaving you with only two at your disposal.

In the Hive, follow the tracks until you find the detonator. Play through the boss fight as usual, then crawl down the hole where the detonator was discovered.
