Call of Duty: Modern Warfare has a lot of difficult situations where you can do harm to civilians. In the mission Piccadilly, if you can avoid shooting any civilians you'll get the Trigger Discipline. But I know an easier way to get this achievement.
First off, load up the mission on Recruit difficulty and run. The mission is very short if you just rush through the stage. When running down the first road, take the subway on the left and kill the terrorist executing the civilians.
Afterward, head outside and go to the opposite wall. Head to the building with the command prompt that will trigger Captain Price's appearance and following the scene. Once done, you'll get the achievement.
This achievement can be found in the mission Embedded. During the final moments of the mission, after blowing up the helicopters, you'll follow Farah into a field of dead bodies. She'll tell you to lay prone and avoid detection. Instead, get up and kill them all. Once they're all dead you'll get the achievement.
