To get the Secret Ending and the Welcome Home trophy, you need to follow a specific path. You won't miss out on anything if you wait, but you must have the 4 Voices before you can finish this. You can even come back to do this after you see the regular ending.

Fixing the Mechanism

Start by heading to the Vault. Look for the place where the Hand caught you earlier in the game. Drop through the floor into the space below and head right in MIO: Memories in Orbit.

At the very end of the tunnel, you will find the Severed Fingertip. Take this to the Lab and go to the room sitting above the checkpoint. Use the part to fix the machine there.

Tracking Down the Overseers

You need to pick up a few items to move forward. Get the Lab Overseer from the left side of the map. After that, go to the large room on the left of the Vault.

Look inside the drawer to find a secret tunnel. Follow the path, hit the pumps, and climb the metal grate. You will find the Vault Overseer sitting right there on the ground in MIO: Memories in Orbit.

Aligning the Shuttle

Go back to the machine in the Lab. Spin it until it hits 0 Degrees. If you drop into the room underneath you, the Vault Shuttle Overseer will be close enough to grab.

Head back up and move the dial to 120 Degrees. Now, use the Lab Overseer to teleport to the Vault Overseer. Go to the old lift shaft and climb all the way up. You can now reach the Vault Shuttle Overseer to turn on fast travel.

Crossing the Crucible

Set the machine back to its start point at 240 Degrees. Use an Overseer from the top of the map to teleport to the Vault Shuttle Overseer. Drop down the shaft to enter the Crucible.

There are two very hard platforming tracks here, one on each side. If you keep dying, go to the menu and turn on Ground Healing. This lets you stand still to heal Armor. Make sure you use the computer on both sides in MIO: Memories in Orbit.

Reaching the True Boss

Once the tracks are done, find Tomo in the center of the Crucible. They will give you Flowing Steps, which let you climb up water. Go to the Canopy and look for a waterfall by the platforms in the middle.

Climb the water to find a hidden area. Head right, take the lift to L-3 Precious Samples, and go to the bridge. Interact with it twice to fight the True Final Boss. Once you win, the trophy is yours.
