If you want to see the secret ending in MIO: Memories in Orbit, you have to find the Crucible. Getting there isn't just for the story; it also gives you the Where You Come To Die trophy.
This area is hidden away, but you can reach it by moving some machines around and finding a few key items.

Repairing the Device
Start your search in the Vault. You need to find the spot where the Hand snatched you earlier in the game. Drop down into the gap below that spot and head to the right in MIO: Memories in Orbit.
Walk until you hit the end of the path to pick up the Severed Fingertip. Once you have it, go to the Lab and find the room right above the checkpoint. Use the part you found to fix the machine there.
Finding the Two Overseers
Next, you need to grab two specific items. Get the Lab Overseer from the section on the left side of the map. After that, head to the big room in the Vault area. Look for a drawer that hides a secret way out.

Go through it and keep moving while you turn on the pumps. Eventually, you will climb up a metal grate and find the Vault Overseer sitting on the floor in MIO: Memories in Orbit.
Moving the Lift
Go back to the machine you fixed in the Lab. Turn the dial until it points to 0 Degrees. Now, drop into the room underneath you to grab the Vault Shuttle Overseer. Go back up and change the machine to 120 Degrees.
Use the Lab Overseer to warp over to the Vault Overseer. Find the main lift you used when you first got here and climb up the empty shaft. This lets you reach the Vault Shuttle Overseer so you can turn on fast travel for it.

Dropping into the Crucible
For the final step, set the machine back to its first spot at 240 Degrees. You need to be in the top half of the map to make this work.
Use any other Overseer to teleport to the Vault Shuttle Overseer. From that spot, jump down the lift shaft. This fall will lead you straight into the Crucible in MIO: Memories in Orbit.
