The secret was reportedly discovered by Discord user Joy2001, and involves the TV in Joel's bedroom displaying a real-life cordyceps infected ant - the fungus that has devastated mankind within the context of the game.

YouTuber Anthony Caliber provides a detailed walkthrough of how to trigger the easter egg.
The player must start a new game on easy, go straight out of the bedroom and down the stairs.
A checkpoint will trigger at the bottom of the stairs, and it's then the player must quit to the main menu of the game immediately after resume their save.
If the player then goes back upstairs and enters Joel's bedroom, the image of the ant will be displayed on the TV.

Kurt Margenau - a developer for the original game - has since commented on the newly discovered image, stating that it may well have been a mistake:

"I could be misremembering 7 years ago but I think the flip book of the ant was like a default shader test thing, and it appearing is probably me not initializing something correctly. Or it was intentional and I don’t remember. I’m old now."
